Thank you for submitting your signed Client Agreement.

Please forward this page to any other legal guardians and anyone who will be present during the services (ie spouse, nanny, significant other, etc) to read & e-sign the DI Agreement before we begin any phone or in-home services. 

​Payment can be sent through Chase QuickPay or Zelle to info@nycpottytraining.com. If QuickPay or Zelle limits the daily amount you can send, partial payment may be sent to hold your date(s) until we receive the remaining amount the following day. A check for the remaining amount can also be mailed to the address at the top of the Client Agreement.
